Registered Nurse (RN)
Full-time and Part-time Home Health RNs to assist our patients in the Metro Atlanta area.
Responsibilities:
- Assess and chart observations of the patient’s condition at each visit.
- Complete evaluation tasks, including reviewing medication and vital signs.
- Administer Physician-prescribed medication.
- Dress or redress wounds and assess the healing progress.
- Provide education to patients and families on proper home health care procedures and strategies.
- Coordinate with Physicians, Physical Therapists, Occupational Therapists, and other individuals in the patient’s care plan.
- Recommend tools or devices that may improve the quality of life for the patient.
Requirements:
- Current RN license
- Home Health experience preferred
- Able to work in the community, drive and meet patients in their home
- Strong organizational and communication skills
Licensed Practical Nurse (LPN)
Responsibilities:
- Provide services within the scope of practice as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team. May not alter the plan of care.
- Complete a progress note for each visit, and submit progress notes to the agency daily.
- Continually observe and report the patient's nursing needs.
- Instruct the patient's family in the care of the patient and maintenance of a healthy environment for the patient.
- Observe, record, and report to the supervising nurse and physician the patient's response to treatment and changes in the patient's condition.
Requirements:
- Current LPN licensure in state of practice.
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
- CPR Certification required.
Certified Medical Assistant (CMA)
Responsibilities
- Complete evaluations, assessments, and treatments in accordance with policy and the individualized care plan
- Complete all documentation and records in a timely and accurate manner
- Follow established policies and procedures as related to the scope of practice
- Maintain continuity of patient care in a home care setting
Requirements:
- Active, current, unrestricted Certified Medical Assistant (CMA) certification
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
- Prior experience in administration.
- Prior experience in medical care.
- CPR Certification is required.
